MacroGenics reports 27% response rate for MGC026 in pretreated head and neck cancer

Six of 22 response-evaluable patients responded, four of them confirmed, and no patient in the 27-person cohort stopped treatment for a treatment-related event. The filing carries no revenue, cash or per-share figures.

MacroGenics reported a 27% best overall response rate for MGC026 in 22 response-evaluable patients with recurrent or metastatic head and neck squamous cell carcinoma, including four confirmed and two unconfirmed partial responses, in interim Phase 1 data presented at the European Society for Medical Oncology (ESMO) 2026 Congress in Madrid and filed with the SEC on October 23, 2026. The filing is a clinical update rather than a quarterly earnings report: it contains no revenue, cash or per-share figures.

The numbers come from Stage 1 of a Simon two-stage expansion cohort in recurrent or metastatic SCCHN, designed to enroll 40 patients at 7.5 mg/kg every three weeks. As of the September 1, 2026 cutoff, 27 patients were evaluable for safety and 22 for response. The release calls them heavily pretreated: 46% had received three prior lines of systemic therapy and 41% four or more, with a median of three (range 1-7). Four of the six responses were confirmed, so the confirmed objective response rate was 18.2% (4/22) and the disease control rate was 64% (14/22). Five of the six responses came from the 16 HPV-positive patients, a 31.3% best response rate in that subgroup. Apostolia Tsimberidou of The University of Texas MD Anderson Cancer Center, the lead principal study investigator, said "of 22 heavily pretreated patients with SCCHN (expansion cohort), six (27.3%) had a partial response."

Blood counts, not lungs or eyes

Across the full study of 85 treated patients, including 32 in dose escalation and 53 in tumor-specific cohorts, 51% had a Grade 3 or higher treatment-related adverse event. Neutropenia (27%) and anemia (25%) were the most common, 5% of patients discontinued for treatment-related reasons, and no Grade 5 treatment-related events were reported. In the 27-patient head and neck cohort the Grade 3 or higher rate was 48%, with anemia at 30% and neutropenia at 26%, and no patient discontinued. The release also states that no cases of interstitial lung disease, pneumonitis or ocular toxicity were observed, the toxicities that have shaped the competitive case for and against other topoisomerase-payload conjugates.

How 27% sits beside 14% to 17%

MacroGenics' comparison slide sets the 27% against best overall response rates of 14% (1/7) for ifinatamab deruxtecan, 17% (5/30) for SYS6043 and 17% (5/30) for SKB500 in unselected late-line head and neck patients. Those are separate trials with separate eligibility rules, and the deck concedes the limit: "No head-to-head clinical trials have been conducted evaluating MGC026 and competing molecules." A cleaner internal comparator is dose escalation, where 17 patients treated at 6 mg/kg or above produced a 29% confirmed response rate (5/17), a 65% disease control rate (11/17) and a median duration of response of 10.2 months.

No revenue, no cash, and two exhibits

Anyone looking for third-quarter financials will not find them in this filing. The report carries Item 8.01, Other Events, and Item 9.01 lists only the press release and the slide deck; there is no income statement, no revenue, no cash balance and no earnings per share. Cash runway appears only as a topic in the forward-looking-statements paragraph, with no figure attached. For a clinical-stage company, that silence leaves the financing question exactly where the last quarterly report left it.

A 2027 readout and a front-line combination

Stage 2 enrollment continues, with updated data expected in mid-2027. Before that, the company plans to open an expansion cohort in early 2027 combining MGC026 with pembrolizumab in front-line, PD-L1 positive (CPS of at least 1) recurrent or metastatic SCCHN; the deck puts that cohort at 40 patients. Eric Risser, the chief executive, said the emerging profile "reinforces our conviction that this molecule has the potential to play an important and differentiated role in the treatment of advanced solid tumors", language the company uses to justify the combination study.
