Intuitive Surgical reported third-quarter 2026 revenue of $3.05 billion, up 22% year-over-year, non-GAAP net income of $1.05 billion or $3.00 per diluted share, and GAAP net income of $522.8 million or $1.49 per diluted share after a $450 million pre-tax antitrust settlement charge, in its earnings release furnished as Exhibit 99.1 to its Form 8-K on October 20, 2026.
As of Tuesday, Oct. 20, 2026.
The operating quarter was the strongest part of the story — the settlement attached to it is what investors sold.
Intuitive Surgical, Inc. (Nasdaq: ISRG) said on Tuesday that revenue rose 22% year-over-year to $3.05 billion in the third quarter ended September 30, 2026, up from $2.51 billion a year earlier. On an adjusted basis the quarter cleared the bar comfortably: non-GAAP net income attributable to Intuitive was $1.05 billion, or $3.00 per diluted share, up from $867 million, or $2.40 per diluted share, a year earlier — ahead of the $2.65 a share consensus, while revenue of $3.05 billion beat a consensus estimate of $2.91 billion, according to Barron's. Reported profit told the other half of the story. GAAP net income attributable to Intuitive fell to $522.8 million, or $1.49 per diluted share, from $704.4 million, or $1.95 per diluted share, because the quarter absorbed the settlement charge detailed below. Investors weighed the charge and a narrower outlook more heavily than the beat: shares plunged 11% in after-hours trading after ending Tuesday's session up 1.1%, according to Barron's.
A settlement five years in the making
In October 2026, following a mediation in the In re: da Vinci Surgical Robot Antitrust Litigation class action, Intuitive reached a written agreement, without any admission of liability or wrongdoing, to settle the previously disclosed antitrust litigation for a one-time payment of $450 million and mutually agreed injunctive relief. The company recorded a $450 million charge to selling, general, and administrative expenses in the third quarter of 2026; the expense was $347 million, net of tax, or $0.99 per diluted share. The settlement remains subject to preliminary and final court approval, and the company cautioned there can be no assurance the court will approve it or that individual class members will not object. Barron's reported the litigation dates back to 2021, when hospital operators accused Intuitive of anticompetitive practices involving the servicing and repair of instruments used with its robotic surgery systems.
Procedures and placements did the heavy lifting
The demand underneath the legal charge kept compounding. Worldwide procedures across the da Vinci and Ion platforms grew approximately 15% year-over-year, with da Vinci procedures up approximately 14% and Ion procedures up approximately 31%. Intuitive placed 546 da Vinci surgical systems in the quarter, including 315 da Vinci 5 systems, against 427 systems, including 240 da Vinci 5 systems, a year earlier; 291 of the placements were under operating lease arrangements, including 160 under usage-based arrangements. It also placed 56 Ion endoluminal systems, up from 50. The installed base reached 12,094 da Vinci systems, up 12% year-over-year, and 1,152 Ion systems, up 21%. Instruments and accessories revenue rose 18% to $1.8 billion, systems revenue was $769.1 million against $590.4 million a year earlier, and service revenue was $482.2 million. Chief Executive Dave Rosa said customers used the company's systems to perform, on average, over 10,000 procedures per day throughout the period.
The guide narrowed at the top and the bottom
For the full year, Intuitive now expects worldwide da Vinci procedure growth of approximately 14% to 15% in 2026 and said it expects to be closer to the midpoint of that range — a narrower band than the previous outlook of 13.5% to 15.5% reported by Barron's, and the kind of trim that gives a richly valued stock no room for error even when the reported quarter beats. The company guided non-GAAP gross profit margin to 68.7% to 69.3% of revenue, a range that includes an estimated tariff impact of 0.9% of revenue excluding recognized tariff refunds, and non-GAAP operating expense growth of 11% to 13%. In the quarter itself, GAAP income from operations fell to $592.6 million from $759.7 million a year earlier on the settlement charge, while non-GAAP income from operations rose to $1.27 billion from $976 million. Both GAAP and non-GAAP results included a benefit of $22 million, net of tax, or $0.06 per diluted share, related to refunds for tariffs paid in prior periods under the International Emergency Economic Powers Act.
Cash went out the door for buybacks, not just the settlement
The balance sheet absorbed the quarter easily. Intuitive ended the third quarter with $6.80 billion in cash, cash equivalents, and investments, down $1.83 billion during the quarter, primarily because cash used for share repurchases and purchases of property, plant, and equipment outran cash generated from operations. The company repurchased 7.8 million shares of its common stock for $2.94 billion in the quarter — a larger cash commitment than the settlement itself, and a reminder that the antitrust payment, if the court approves it, lands on a company still generating cash faster than the charge consumes it.
What has to clear next
What to watch is whether the settlement wins preliminary and final court approval without objections that reopen it, whether fourth-quarter da Vinci procedure growth tracks the narrowed 14% to 15% full-year band near its midpoint, and whether da Vinci 5 keeps taking a rising share of placements as the installed base compounds. The antitrust overhang does not disappear with the charge — it disappears with a judge's signature — and until then the stock will trade on both stories at once: a procedure engine growing in the mid-teens, and a settlement that still has to clear.
