CVS Health reported adjusted earnings of $2.09 a share for the third quarter, beating the $1.65 that analysts polled by FactSet had expected.

CVS Health Corporation combines the Aetna health insurer, a pharmacy benefit manager, and a national retail pharmacy chain. Its shares trade on the New York Stock Exchange as CVS. The company reported results for the three months ended September 30, 2026.

The quarter beat expectations

Adjusted earnings per share were $2.09 for the quarter. A year earlier, adjusted earnings were $1.60 a share. The release tied the increase mainly to Aetna. It cited improved adjusted operating income in Health Care Benefits. That segment is executing a margin recovery plan, the release said.

GAAP diluted earnings per share were $1.68. A year earlier, the company lost $3.13 a share. That loss included a $5.7 billion goodwill impairment charge. The charge sat in the Health Care Delivery reporting unit. No such charge recurred this year.

Total revenues rose 5.4% to $108.4 billion. Growth came from Health Services and Health Care Benefits. Net income was $2.17 billion, against a $3.99 billion loss a year earlier. Operating income was $3.57 billion. Adjusted operating income rose 21.2% to $4.19 billion.

Chief Executive David Joyner credited the workforce in the release. "Through their continued dedication, each of our businesses delivered another quarter of strong results," he said.

Aetna's recovery did the heavy lifting

Health Care Benefits houses the Aetna insurer. Its revenues rose 2.4% to $36.86 billion. Growth in the Government business drove the increase. It was partly offset by the 2026 exit from the individual exchange business.

Adjusted operating income nearly tripled, to $912 million. A year earlier it was $314 million. The release credited improved underlying Government business performance.

The medical benefit ratio, the share of premiums paid as medical costs, improved. It fell to 91.3% from 92.8% a year earlier. That is a 1.5-percentage-point improvement. Medical membership was 26.1 million at September 30. The release called that consistent with June 30.

Two more markers pointed the same way. Prior years' health care cost estimates developed favorably by $1.3 billion over nine months. Days claims payable were 41.9 days at September 30. More than 69% of Aetna Medicare Advantage members sit in 2027 plans rated four stars or higher. Aetna ranks second among large publicly traded Medicare Advantage organizations, the release said.

Health Services grew fastest

Health Services runs the pharmacy benefit manager and clinic services. Its revenues rose 9.5% to $53.96 billion. Pharmacy drug mix and brand inflation drove the growth. Continued client price improvements partly offset it.

Adjusted operating income rose 2.7% to $2.11 billion. Improved purchasing economics and drug mix helped. Client price improvements partly offset those gains. The segment processed 481.7 million pharmacy claims. That was up 1.3% on a 30-day equivalent basis.

The pharmacies earned more on less revenue

Pharmacy & Consumer Wellness runs the retail pharmacies. Its revenues fell 2.0% to $35.50 billion. Regulatory price reductions on certain drugs weighed on the total. Recent generic introductions and reimbursement pressure also weighed.

Adjusted operating income still rose 10.6%, to $1.64 billion. Core pharmacy strength drove the increase. Contributions from the Rite Aid asset acquisitions also helped. Business investments and consumer dynamics partly offset the gains. Prescriptions filled rose 1.9% on a 30-day equivalent basis, to 470 million.

Guidance rose, with caution attached

CVS announced full-year 2026 revenue guidance of at least $420.0 billion. That is up from at least $414.0 billion previously. Adjusted EPS guidance rose to at least $8.20 a share. The prior range was $7.90 to $8.10. FactSet analysts had expected $8.05, per the Journal.

GAAP diluted EPS guidance moved to at least $6.98, from a range of $6.84 to $7.04. The release used that exact framing for the GAAP line. Cash flow from operations guidance rose to at least $15.5 billion. It was at least $11.5 billion before.

The company tied the raise to two segments. Health Care Benefits and Pharmacy & Consumer Wellness both improved. It kept a cautious view for the rest of the year. Elevated cost trends and possible macro headwinds were the reasons given.

Year-to-date figures frame the remaining task. Nine-month revenues were $314.92 billion. Nine-month GAAP diluted EPS were $6.29, and adjusted EPS were $7.24. Cash flow from operations reached $15.3 billion over the nine months.

Shares eased in premarket trading

CVS shares closed on November 3 at $91.41, up 3.28%. In premarket trading on November 4, they eased to $89.98. That was a decline of 1.56% as of 7:03 a.m. Eastern, per Nasdaq. Premarket trading is thin, so the opening print may differ. The snapshot is a premarket print, not a close.

The earnings release did not address the reported pharmacy-separation plans, which TickerGrove covered on November 3, and the company's 8:00 a.m. Eastern call was still ahead.

What remains unknown

What remains unknown starts with the medical cost trend. The release still calls cost trends elevated. Whether Aetna's margin recovery holds at that cost level is untested. The favorable development of prior cost estimates may not repeat.

The guidance also leaves the cautious view unquantified. The release does not size the macro headwinds it cites. Nor does it split the raise between its two named segments.

What to watch next

What to watch next starts with the earnings call. Watch the 8:00 a.m. Eastern call for cost-trend detail. Watch whether management quantifies the caution in its raise.

Watch the cash session against the premarket print. Watch fourth-quarter medical costs against the recovery plan. Watch membership when the next quarter reports.
