Centene Beats Estimates and Raises Its Full-Year Earnings Guidance
Better-than-expected medical costs in Medicare powered the beat, and management now expects a stronger finish to 2026.
Centene posted adjusted earnings of $0.58 a share for the third quarter, far above the $0.10 consensus, and raised its full-year earnings guidance.
Centene Corporation reported its third-quarter results on Oct. 27, 2026. The quarter covered the three months ended Sept. 30, 2026.
Centene runs government-sponsored health plans. Its lines cover Medicaid, Medicare and Marketplace members.
A clear beat against low expectations
Adjusted diluted earnings were $0.58 a share, up from $0.50 a year earlier.
Seeking Alpha reported that the result beat consensus by $0.48 a share. Its tally implies consensus of $0.10 a share, a TickerGrove calculation.
The beat was wide, not marginal. Seeking Alpha reported that revenue of $50.65 billion beat consensus by about $3.7 billion.
Under generally accepted accounting principles, the company posted a diluted loss of $0.14 a share.
A year earlier the loss was $13.50 a share. That loss reflected a goodwill impairment of $6.72 billion in the 2025 quarter.
The adjusted result excludes amortization, severance and other items. The release reconciles the gap between the two measures.
Revenue grew as membership shifted
Total revenues were $50.65 billion, up 1.9% from $49.69 billion a year earlier.
Premium and service revenues were $45.11 billion, up 0.5%.
Premium revenue by line was $24.04 billion for Medicaid and $8.7 billion for Commercial. Medicare premium revenue was $11.26 billion, up 20%.
Total at-risk membership was about 25.7 million at Sept. 30. A year earlier it was about 28.0 million.
Medicaid membership was about 11.9 million. Marketplace membership fell to about 3.4 million from about 5.8 million.
Medicare Prescription Drug Plan (PDP) membership rose to about 8.9 million. A year earlier it was about 8.0 million.
Medical costs landed better than expected
The health benefits ratio (HBR), the share of premiums spent on medical care, was 92.7%.
The ratio was unchanged from a year earlier. Centene said it was better than its expectations.
The company credited Medicare for the strength, driven by its Medicare Advantage and prescription drug businesses. It said Medicaid and Marketplace were on track.
Centene also cited improved Marketplace pricing. Risk transfers reflected the acuity of its Marketplace membership.
"Solid underlying fundamentals within our core businesses have powered a faster recovery to our earnings trajectory than previously anticipated," said Chief Executive Sarah M. London.
Guidance rose, and the cost guide fell
Centene raised its 2026 adjusted diluted earnings floor to greater than $5.20 a share. The company said the increase was $0.40.
The previous floor was greater than $4.80 a share.
The floor for diluted earnings under generally accepted accounting principles rose to greater than $3.46 a share.
Full-year total revenue guidance is $196.5 billion to $200.5 billion. Premium and service revenue guidance is $173.0 billion to $177.0 billion.
Centene lowered its full-year HBR guidance to 90.4% to 91.1%. The midpoint is 90.75%, an improvement of 15 basis points, or 0.15 percentage points.
Interest expense guidance also fell, to $600 million to $620 million.
Debt fell while operating cash flow stayed negative
Operating cash flow used in the quarter was $3.67 billion. Centene attributed it mainly to the timing of pass-through, premium and other payments, as expected.
Cash, investments and restricted deposits were about $39.9 billion at Sept. 30. Days in claims payable was 48, up one day from the second quarter.
Total debt was $15.5 billion after third-quarter repurchases of senior notes. Centene said debt fell $1.1 billion from June 30, 2026.
In October the company increased its debt repurchase program by $2.0 billion. It then repurchased the remaining $569 million of senior notes due 2027.
That completed the full redemption of the 2027 senior notes, the company said. About $1.9 billion remained available under the program on Oct. 27.
What remains unknown
What remains unknown is how membership and medical costs develop in 2027, because this release sets guidance only for 2026. Also unknown is whether the better Medicare performance repeats in the fourth quarter. What to watch is fourth-quarter results against the raised floor, any 2027 guidance from management, and whether Marketplace membership stabilizes after its sharp fall. The verified record is clear on the quarter itself: earnings beat low expectations by a wide margin, medical costs landed better than the company expected, and management raised the year's earnings floor.
